Site Nurse (LPN)
Position Title: Site Nurse
Position Location: Franklin County (various locations in Columbus, OH)
Job Summary: The Site Nurse is responsible for the coordination of all health and medical services provided to the consumers who have a developmental disability and live in various settings in Franklin County. This care is provided under the direction of the assigned Registered Nurse. The primary areas of responsibility includes a knowledge of all agency’s policies and procedures, an evaluation of the consumers’ needs, and the development of health service coordination plan that identifies needed services, and supports, and then finally ensure that the plan is appropriately implemented.
The site nurse participates in ISP meetings, oversees the administration of medication and medical treatments per physician’s orders, and communicates with home staff, team members, physicians, and others as needed, and ensures that proper documentation of medication ant treatments are administered. The site nurse will also train all direct care staff as required or requested.
Position Requirements: The candidate must be a graduate of an accredited program and hold a current license in the State of Ohio as a Licensed Practical Nurse (LPN). In addition, due to the independent nature of the duties required, candidates must have a minimum of 1 year of experience, preferably in long term care or with individuals who have a developmental disability. Other requirements include a current Ohio Driver’s License, and excellent verbal and written communication skills. Applicant must possess strong organizational skills and attention to detail.
